Submit your updated insurance certificate at least 11 days before the current Certificate of Insurance (COI) expires.

Our records show that the insured -INSERT CARRIER LEGAL NAME- with offices located at -INSERT CITY, STATE- and operating under the USDOT number of -INSERT USDOT#-, has insurance policies that are currently expired or will be expiring soon. Per the terms of the contract the insured has entered into with EmergeTech, Inc., an updated insurance certificate naming EmergeTech, Inc. as a certificate holder must be submitted to the email address shown below. An expedited processing of this request is necessary to prevent further disruption of the insureds account and restore their ability to operated within our network as a motor carrier.
